    Any person wanting to obtain a certificate or permit to transport passengers for compensation, by motor vehicle, over any public highway of this State, must submit this application with exhibits . A certificate or permit issued by the PUC is required under Section 271-8, HRS except for persons transporting passengers without charge, operating school buses and other transportation means as provided under Section 271-5, HRS.  Required exhibits include:

      • Exhibit A-Partnership or Corporation Information (Form 92-002)
      • Exhibit B-Vehicle Inventory List (Form 92-003)
      • Exhibit C-Balance Sheet (Form 92-004)
      • Exhibit D-Fixed Assets Schedule (Form 92-005)
      • Exhibit E-Loans Payable Schedule (Form 92-006)
      • Exhibit F-Projected PUC Operating Revenues & Statistics (Form 92-007)
      • Exhibit G-Projected Income Statement (Form 92-008)
