This document provides general instructions for completing the State of Hawaii's tax clearance application form.

Any motor carrier obtaining a certificate or permit to transport passengers or property or to transfer such certificate, permit or carrier property must submit this application to the Department of Taxation. A tax clearance must accompany all motor carrier applications in order for the application to be filed at the PUC.
