Any motor carrier wanting to transfer a certificate, permit or property must submit this application with exhibits . A PUC approval via application for such transfers is required under Section 271-18, HRS.  Required exhibits include:

• Exhibit A-Partnership or Corporation Information (Form 92-002)
• Exhibit B-Vehicle Inventory List (Form 92-003)
• Exhibit C-Balance Sheet (Form 92-004)
• Exhibit D-Fixed Assets Schedule (Form 92-005)
• Exhibit E-Loans Payable Schedule (Form 92-006)
