Application for Motor Carrier Certificates or Permits for Transportation Passengers (Form 92-001)

Any person wanting to obtain a certificate or permit to transport passengers for compensation, by motor vehicle, over any public highway of this State, must submit this application with exhibits . A certificate or permit issued by the PUC is required under Section 271-8, HRS except for persons transporting passengers without charge, operating school buses and other transportation means as provided under Section 271-5, HRS.

    Your motor carrier application will NOT be accepted for filing at the Public Utilities Commission unless it is accompanied by a Tax Clearance Certificate issued by the Department of Taxation.

    1. Go to forms on the Department of Taxation web site (https://tax.hawaii.gov/forms/) and download Form A-6 and the Instructions for Filing Forma A-6.
    2. Read the instructions and complete the appropriate sections of Form A-6.
    3. At line 4, enter a check mark in the box and enter the words “State PUC Motor Carrier Licensing” after the word “Others”.
    4. Take the form to the Department of Taxation and obtain a Tax Clearance Certificate.

  1. Individuals seeking to eFile must register online.
    If you are not registered visit the Electronic Filing (eFiling) page. Within 2 business days of registering you will receive an email to active your account.
  2. Your motor carrier application will NOT be accepted for filing at the Public Utilities Commission unless it is accompanied by a Tax Clearance Certificate issued by the Department of Taxation.  See General Information (PDF) document for more information.

  2. Follow the online form’s instructions to complete form and submit to the PUC.

  3. Go to the Filing Docketed Documents eform.
  4. Follow the online form’s instructions to complete form and submit to the PUC.
    1. Set Document Category: Applications, Petitions, and other initiating documents.
    2. Set Document Type: CPCN/Permit-New Motor Carrier.
    3. Review the Requirements to File Confidential or Restricted Documents. (most motor carrier applications are not confidential and are available for the public)
    4. Attach your application and supporting documents (e.g. Tax Clearance Certificate) by clicking the “Attach…” button(s). You may upload the application and supplemental documents as a single or separate files.
