Application for Motor Carrier Certificates or Permits for Transportation Passengers (Form 92-001)
Any person wanting to obtain a certificate or permit to transport passengers for compensation, by motor vehicle, over any public highway of this State, must submit this application with exhibits . A certificate or permit issued by the PUC is required under Section 271-8, HRS except for persons transporting passengers without charge, operating school buses and other transportation means as provided under Section 271-5, HRS.

eFile (Electronic File)
- Individuals seeking to eFile must register online.
If you are not registered visit the Electronic Filing (eFiling) page. Within 2 business days of registering you will receive an email to active your account. - Your motor carrier application will NOT be accepted for filing at the Public Utilities Commission unless it is accompanied by a Tax Clearance Certificate issued by the Department of Taxation.
