Motor Carrier Insurance Forms

Passengers & Property

The “Uniform Motor Carrier Bodily Injury and Property Damage Liability Endorsement” and “Uniform Motor Carrier Bodily Injury and Property Damage Liability Certificate of Insurance” forms must be submitted by motor carriers for property and passengers under the following conditions (under Sections 6-62-8 and 6-62-12 of the Hawaii Administrative Rules):

  1. When obtaining a new certificate or permit;
  2. When reactivating an existing certificate or permit;
  3. When transferring an existing certificate or permit;
  4. When changing insurance carriers; or
  5. When reinstating a suspended/cancelled certificate or permit.

 

Property

The “Hawaii Motor Carrier Cargo Certificate of Insurance” and “Hawaii Motor Carrier Cargo Insurance Endorsement” forms must be submitted by motor carriers for property, excluding dump truck carriers, under the following conditions (under Section 6-62-9 and 6-62-13 of the Hawaii Administrative Rules):

  1. When obtaining a new certificate or permit;
  2. When reactivating an existing certificate or permit;
  3. When transferring an existing certificate or permit;
  4. When changing insurance carriers; or
  5. When reinstating a suspended/cancelled certificate or permit.
